The John Brockington Foundation Celebrity Golf

Classic at La Costa Resort and Spa Sponsorship

Opportunities

Thank you National University Systems and all of our sponsors for making our 1st year tournament a success. We would like to also thank everyone who participated in this year’s tournament. We look forward to bringing you an even more fun and exciting event next year in 2013. Until then, thank you for all your support of the John Brockington Foundation.

The mission of the John Brockington Foundation is to create a culture in which organ donation is commonplace; to provide financial and resource support for those donating, awaiting, and/or receiving organs; and to promote health education to minority communities who are disproportionately represented on the transplant waiting list.

Our programs assist San Diegans every step along the continuum of chronic kidney disease:

Educate:
• Keeping Inside the Numbers (KIN) provides culturally relevant education, screenings, and self-management tools to assist the South East San Diego community where chronic kidney disease (CKD) is 44 times more prevalent than in other communities. KIN reduces modifiable risk factors such as high blood pressure, high cholesterol, obesity, physical inactivity, and poor nutrition by having like presenters help create behavior change in those most at risk. KIN maximizes the impact of our funding by targeting a specific community and directly addresses root causes that affect the health of its members.

• CKD community education classes: In partnership with Balboa Nephrology Medical Group, JBF offers free monthly Medicare funded kidney education classes throughout the year for Stage IV CKD patients referred by BNMG’s 35 physicians.

Sustain:
Our food vouchers help sustain protein levels in dialysis patients in 34 clinics in San Diego County, serving over 3,000 patients.

Transplant:
JBF co-founded Donate Life San Diego which helped create and promote California’s online organ donor registry in 2004 and authored the Workplace e-campaign adopted by California (2009) and Arizona (2010). The FanForLife organ donor registration campaign runs until April 28th, 2012.

JBF programs in the last 12 months:

• Free Medicare funded kidney education classes offered twice a month throughout the year in partnership with Balboa Nephrology Medical Group.
• Over 120 high-risk residents of South East San Diego screened through our KIN events.
• $35,000 in food vouchers distributed to 700 dialysis patients, bringing the total to $164,500.
• Over 8 million Californians now registered on www.donateLIFEcalifornia.org, in part through the JBF/Lifesharing e-campaign adopted by California and Arizona.
